If possible, please have the senior participate in the Senior Solve Assessment. If this is not possible, or is problematic, just do the best you can and I will guide you in gathering more information during our telephone conference.
The assessment is an eight part form that gathers the following information:
- Basic personal information
- Daily activities
- Household activities
- Mental Well-Being
- Personal Appearance and Environment
- Health History and current status
- Medications
- Conclusion
After submitting the evaluation, the screen will confirm that it has been successfully submitted and you will immediately receive an e-mail confirmation.
I will telephone you within three business days and the creation of the care plan will commence. The care plan will be completed and then discussed with you within seven business days.
